Lee explained that while the game includes a feature where male and female Zois can be together with the intention of creating babies, the visual representation of this act will be left to the player's imagination. He stated, "What you're seeing is not what you're seeing; you have to assume that's what's happening." The developers also discussed the challenges of handling nudity and explicit content within the game's realistic art style. For instance, they noted that pixelated blurring effects, commonly used in more cartoony games, felt too suggestive when applied to inZOI's visuals. An example given was the issue of Zois showering with towels on, as attempts to use blurring effects resulted in unintended nudity during mirror reflections, a bug that was subsequently fixed.
Despite these discussions around sex and nudity, inZOI has been rated T for Teen by the ESRB, indicating its intention to cater to a wide audience while maintaining a safe-for-work environment.
